All applicants are required to complete the application process and submit each of the following by 21 days (3 weeks) prior to the date of the examination:
1. A Perioperative Blood Management Technologist Examination application completed in full.
2. Verification documents of HS Graduation or advanced degrees and certifications, completed clinical activity reports sent to the IBBM Office.
3. An application fee of $200.00 USD.
